State of Grace PLLC is an In-Network provider for Blue Cross Blue Shield (BCBS), United Health and Aetna.

Out-of-Network insurance documentation can also be provided for your submission and reimbursement.

Additionally, payment is accepted through HSA’s, FSA’s, Debit/Credit, Cash or Check

Many people prefer to use the private pay option because they are not required to be labeled with a mental health diagnosis, or have medical information disclosed in order to receive counseling services.

Insurance: Services may be covered in full or in part by your health insurance or employee benefit plan, please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?

• What is my deductible and has it been met?

• How many sessions per year does my health insurance cover?

• What is the coverage amount per therapy session?

• Is approval required from my primary care physician?

Availability: Evening and weekend sessions available by appointment

Cancellation: I request and require 24-hour notice if you are unable to attend your scheduled appointment. If you miss more than 3 appointments without notification we will have to discuss discharge.
